Korte omschrijving:
Bij de ziekte van Dupuytren ontstaan er verdikkingen en verkortingen in de bindweefselplaat, die net onder de huid van de handpalm ligt.
Hierdoor buigen de vingers naar de handpalm toe en kunnen ze niet volledig meer gestrekt worden.
Uiteindelijk wordt de beweeglijkheid van de vinger(s) beperkt.
Het gevolg is een continu gebogen stand van de vingers (contractuur), vooral de 3de t/m de 5de vinger.
Deze aandoening kan zich ook voordoen op andere plaatsen van het lichaam, zoals de knokkels van de vingers of de voetzolen.
Het eerste teken van de ziekte van Dupuytren is een klein knobbeltje of kuiltje in de handpalm, vlakbij de handlijn aan de basis van de ringvinger en de pink. 
Geleidelijk aan kan zich een streng gaan ontwikkelen tussen de palm en de vingers. De afwijking wordt vaak pas opgemerkt als de hand niet meer vlak geplaatst kan worden op een tafelblad.
De afwijking veroorzaakt geen pijn, maar kan op den duur ernstig functieverlies teweegbrengen, zoals bij het handen wassen, het handschoenen dragen en het in de zakken steken van de handen.

A lump or pit (nodule) may show up months or even years before a cord develops from the palm to the fingers.


Oorzaak:
 

De oorzaak van de ziekte van Dupuytren is onbekend.
De ziekte is niet te genezen, maar wel te behandelen.
Chronische belasting kan evenwel een rol spelen bij het ontstaan.

There is no permanent cure for Dupuytren's contracture.
Progress of the disease is sporadic and unpredictable.
Exactly what triggers the formation of cords is unknown.
If the feet are involved the prognosis is worse.
As the disease progresses, diseased tissue wraps itself around & between normal tissue.
Many people do not seek medical care until the contracture is well advanced.
Surgery is the treatment for this condition.
Surgery is usually reserved for individuals who have developed deformity with restricted finger motion as a result of the progressive contracture.

Ethiologie:
Behoort tot specifieke CANS. Komt het meest voor boven 40 jaar en meer bij mannen dan bij vrouwen.

Diagnose:
Signs and Symptoms Dupuytren's Contracture Occurs slowly.
First sign is a thickening (nodule or cord), or skin puckers or pits, and gradually a cord develops between the palm & fingers extending into the finger.
Nodule most frequently develops in the palm to near the crease of the hand closest to the base (knuckle) of the ring and/or little finger (description of injury above). Nodule or Cord are painless, but can be tender to the touch
Stiffness in the palm of hand and fingers with inability to straighten fingers
Deformity the fingers are bent (flexed) in toward the palm.
Ring finger is usually affected first, followed by the little/pinky, long and index fingers. Usually noticed when the palm cannot be placed flat on an even surface, such as a table top.
Problem is restricted motion & the deformity it causes--- NOT pain!!

Chirurgische behandeling:
Deze bestaat uit het operatief verwijderen van het abnormale weefsel aan het peesblad, waarna de vingers weer gestrekt kunnen worden.

 

Because many nodules do not progress to cords, or contracture and because scar tissue from previous surgeries can make excision of recurrent nodules more difficult, surgical removal of isolated nodules is not indicated in most cases.
Presence of the lump (nodule) in the hand does not mean that surgery is required. Bending of the fingers into the palm at the base of the fingers is usually correctable by surgical release of the connective tissue (fascia in the palm of the hand/fingers), but the condition can return with time.
Goal of surgery is to restore the use of the fingers by releasing the contracture and improve hand function by releasing & removing the diseased tissue.
May be required for troublesome stiffening and loss of motion that's unresponsive to other treatments.
Surgery may not completely relieve bending of the fingers at the joints in the fingers.

Niet-chirurgische behandeling:
De percutane fasciotomie of naaldaponeurotomie is afkomstig uit Frankrijk waar de reumatologen deze techniek al jaren toepassen voor de behandeling van de ziekte van Dupuytren.

Deze niet-chirurgische behandeling van Dupuytren gebeurt tijdens een normale consultatie en duurt ongeveer dertig minuten. Het aantal sessies die nodig zijn, hangt af van de graad van evolutie waarin de ziekte zich bevindt.
 

De techniek berust op een mechanisch doorhalen van de fibrotische strengen ter hoogte van de handpalm door middel van het stervormig op en neer bewegen van de naald.

Algemene adviezen/preventie:
Risk factors for Dupuytren's Contracture:

 

  - People who are: White of northern European descent (Scandinavian, Irish, or eastern European; there is a genetic link as the disease runs in families.)
- Men--Is about seven times more common in men than in wome
- Over age 40--Most of the time, doesn't show up until after age 60. A very severe form may appear in teenagers & children.
- Diagnosed with Diabetes, Alcoholics, and Taking anticonvulsant drugs for epilepsy.
